For all bid and performance bonds including site improvement
bonds, please provide: 1. Fully completed application. 2. Latest business and personal tax returns - first
five pages only. 3. Latest business year end financial statement prepared by a CPA if amount of request is in
excess of $250,000 4. Personal balance sheet. 5. Details on project to include overall bid amount, time to
complete, delay damages - if any and term of warranty when completed.
